Clinic Documentation

Please use the following templates when completing visit documentation:

  • DEVPEDSNEW: A&DD, Down Syndrome, and Neuromotor new patients
  • DEVPEDSFOLLOW: A&DD, Down Syndrome, and Neuromotor follow up
  • DEVPEDSASDEVAL: A&DD autism diagnostic evaluations
  • NBFU CLINIC: NBFU patients

Documentation should ideally be completed on the same day of the visit. If that is not feasible due to other commitments, discuss the plan to complete documentation with attending.

Consider the following guidance for documentation:

-Write clinic notes for families! We send a copy of all of our notes to our patients’ families.  Families use these notes for treatment planning, working with therapist/school, and applying for other services and supports so our notes look different than a typical medical note

-Avoid judgmental or accusatory language (e.g. instead of saying “Mother denies”, say “Mother reports no concerns about”)

-If a patient or family shares their preference regarding use of Person-First language (e.g. child with autism spectrum disorder) versus identity-first language (autistic child) please respect that preference, both in verbal communication and documentation. Always avoid use of derogatory terms such as mental retardation or MR.

-Minimize medical jargon and abbreviations- especially in recommendations

-Include parent-report regarding development/behavior in the HPI and your own observations of development/behavior in the Physical Exam/Observation section

-Write recommendations as instructions to families
