In lecture of Week 2, show the instructor that you have completed the forms for your background check.  
Then, bring the form to your site when do perform your site visit; your site will keep the form and is responsible for running your background check.
If you are volunteering at most all sites (associated with MMSD), you need to fill out just this one form, pages 4-5:
If you are volunteering at the Salvation Army, you have these forms to fill out:

If you will be at MCM, fill out the MMSD form above to show me, but MCM will have additional forms for you to fill out during your site visit.
