Reporting Incidents
General Reporting
As part of the Student Services team, Mark Matosian (mark.matosian@wisc.edu) and Nicole Kaforski (nicole.kaforski@wisc.edu) welcome students to disclose any concerns or incidents you may have experienced. Reported incidents are taken seriously - All students are expected to adhere to the Community Guidelines. When you report an incident, we will discuss your wellbeing and needs, provide as much support and information as we are able, and work with you to determine the steps you would like to take next. We are committed to investigate and take action where we are able, to support students and address immediate needs to the best of our ability, and to connect students to supportive campus and community resources. We are also here if you simply need to reflect, process, have a safe space, or need to be connected with a campus resource.
Mark and Nicole are located in Suite 2310 of Grainger Hall.
Please note, other staff and faculty are also reliable resources to report incidents to. We encourage you to work with a member of the WSB or UW-Madison community that you feel comfortable with.
Anonymous Reporting
Per University policy, the Student Services team, and other staff and faculty, are required to report incidents around sexual harassment, sexual assault, dating/domestic violence, stalking, sexual exploitation and related retaliation. This information is reported directly to the Sexual Misconduct Resource and Response Program.
For other incidents: If you wish to report an incident anonymously about a fellow student, we may be limited in our ability to take action. This is in part for keeping your identity and experience confidential, as well as our inability to provide the person who committed the act with specifics around the incident that occurred. We will work with you to discuss the steps you feel comfortable taking. The Student Services Team will also create a report on the incident.
Behavioral Misconduct
Students who do not adhere to the Community Guidelines and program requirements, or demonstrate a pattern of unacceptable behavior, will meet with the Honor Board to discuss their actions. The Honor Board will determine next steps or consequential actions, which may include removal from the program.
If a reported incident needs to be escalated beyond the Student Services Team, or you wish to report an incident independently, the Dean of Student's Office has a comprehensive list of reporting lines. Each office's goal is to support you and inform you through each reporting process.
Examples of incidents you have the ability to report include hate, bias, sexual assault, accessibility barriers, students you are concerned about and other types of incidents.

Sexual Misconduct Resource and Response Program
The Sexual Misconduct Resource and Response Program (formerly called the Title IX Program) is overseen by the Title IX Coordinator. This office receives reports of sexual harassment and sexual violence – including sexual assault, dating/domestic violence, stalking, and sexual exploitation – and coordinates the University’s response. The Student Services team is required to report incidents around sexual harassment, sexual assault, dating/domestic violence, stalking, sexual exploitation and related retaliation.
Reports of sexual misconduct are taken seriously. In addition to the Student Service's Office, the Sexual Misconduct Resource and Response Program office's goal is to support you and inform you through your reporting process.
