Pre-Course Questionnaire

Steps for this assignment are as follows:

• Download the questionnaire pdf file Download questionnaire pdf file to a computer (there are known issues with mobile devices)
• Open the file in Acrobat Reader DC (if you don't have it, get it free here Links to an external site.)
• Fill in the blue form fields as directed on the questionnaire
• Save your work with the following filename: Lastname_Firstname.pdf
• Close Reader DC and reopen your saved file to ensure your information saved correctly
• Upload the pdf file to this dropbox by 5:00 pm on Friday, September 3
